- One proof of residential address (utility bill, lease, bank statement)
Sign your form
USPS requires a signed Form 1583 for every mailbox. Sign it in person at our store (free, takes 5 minutes) — or get it notarized elsewhere and mail it to us.

Why notarization is required
Federal mail regulations require anyone renting a private mailbox to sign USPS Form 1583, with two forms of ID on file. If you sign that form in front of us in the store, no notary is needed — we witness it ourselves. If you can't come in (you're out of state, or it's easier to do it locally), have it notarized wherever you are and mail or drop off the signed, notarized copy along with copies of your two IDs.
This is a one-time step — once it's on file, you won't need to redo it unless your ID expires or your information changes.
